The NCAA mandates that documentation for any student-athlete being medicinally treated for ADD or ADHD be filed confidentially in the Sports Medicine Dept. This downloadable document must be signed by the student-athlete's treating physician and must be on file in the Sports Medicine department in the event the individual tests positive during an NCAA drug test. If this documentation is not on file prior to testing positive they may not be granted a Medical Exemption. The form needs to be completed every year by all student-athletes being treated for ADD or ADHD.
