The Sports Medicine staff is comprised of four Certified Athletic Trainers who work very closely with the University of Massachusetts Medical School Sports Medicine Faculty. Dr. Lee Mancini serves as Assumption College's Intercollegiate Team Physician.
The Sports Medicine offices and the athletic training room are located on the lower level of the Laska Gymnasium. If you have any questions, feel free to contact the Assumption College Sports Medicine Staff.
Assumption College Returning Student-Athlete Forms
All returning student athletes are required to fill out Sports Medicine, Communications and Compliance paperwork on ARMS. You will receive an email from ARMS prior to the start of your athletics season. Please make sure to fill out ALL forms completely. Please also complete the forms below:
Assumption College First-Year/Transfer Student-Athlete Forms
All first-year and transfer students athletes are required to fill out Sports Medicine, Communications and Compliance paperwork on ARMS. You will receive an email from ARMS once you have an established Assumption email account. Please make sure to fill out ALL forms completely.

- Department Accident Insurance Policy
All first year student-athletes are required to fill out the student-athlete health report form before he/she may begin their initial season of competition at the college. Student athletes who are transferring to Assumption, or took more than one full semester off must also complete this form. The first page has information and a checklist regarding all of the first year requirements.
